Medicare Part C is what is known as "Medicare Advantage" and is one of the savings plan options offered by Medicare when you have already enrolled in Original Medicare.

The enrollment form is different from Original Medicare, since you must already have coverage in these two parts to enroll in a Medicare Advantage or Part C plan.

And also the name of your general practitioner and specialist, to verify if the plans also have these doctors in their network.

Supplemental plans have a monthly premium unlike Medicare Advantage plans, which generally do not have a monthly premium.
