Special Needs Plans (SNP) are Medicare Advantage plans that have specific requirements for members. There are plans for people with certain chronic conditions, people who are dual eligible for Medicare and Medicaid, and people who live in institutions. Each of these types of plans is designed to meet the needs of its members. Special Needs Plans (SNPs) are created to meet the needs of your plan members. There are three types of SNPs and each is restricted to individuals who meet certain criteria.<\/p>

Chronic Condition SNPs (C-SNPs) are for people who have one or more chronic conditions as defined by the Centers for Medicare & Medicaid Services (CMS). These SNPs are designed to help provide their members with the health care they need. That includes access to treatment and coverage for their conditions and the ability to access a network of health care professionals who can treat their condition.<\/p>

Dual Eligible SNPs (D-SNPs) are for people who are eligible for both Medicare and Medicaid. These SNPs give their members access to a network of health care providers who accept Medicaid coverage.<\/p>

Institutional SNPs (I-SNPs) are for people who live in an institution such as a skilled nursing facility or adult disability facility on a permanent basis (at least 90 days). These plans provide their members with all the coverage needed for this population.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

As a type of Medicare Advantage plan, special needs plans have all the coverage you get from Medicare Part A and Part B. This includes medical and hospital insurance, which covers inpatient care in a hospital or skilled nursing facility, hospice care, home health care, preventive care and medically necessary services. For your SNP, you will pay premiums, an annual deductible, copayments and coinsurance. Prescription drug costs will work the same way as in a Part D plan. You will pay copayments based on whether your prescription is generic or brand name, preferred or non-preferred, or a specialty drug. You will also continue to pay your monthly Part A and Part B premiums, as with all Medicare Advantage plans.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Special needs plans require that you have a primary care physician or care coordinator. You must also get a referral to see a specialist in most cases.<\/p>

These plans have networks of health care providers that are tailored to the needs of these groups. The ability for people with chronic conditions to visit specialists and health care providers who can treat their conditions is a key benefit of C-SNPs. D-SNPs offer low prescription drug costs and special financing options, ensuring that their providers accept Medicaid coverage.
